Texas High School Salsa Championship Release forms are a requirement for participation.
Each student, teacher and chaperone participating in the the workshop,
competition and salsa social is required to submit a completed release
form.

| I hereby release the teachers, staff, volunteers, promoters, and judges of the Texas High School Salsa Championship, the Texas Association for Hispanic Dance and Culture, and the Dallas Community College District from any and all claims for damages, injuries, or loss to my person or property during my participation in the Texas Salsa Championship. I further release the teachers, staff, promoters, volunteers, and judges from any liability for injury, damage, or loss which may have been caused by any act or omission. | |
By signing the release form the participant named below gives permission to use his or her photo image or or video recording for promotion of the Texas Salsa Championship, and for promotion and advancement of Hispanic Dance by the Texas Association for Hispanic Dance and Culture. |

| By signing, each of the participants agrees to be bound by the waiver and release agreement. Participants have read the rules and regulations of the competition and agree to each. The signature of the parent or guardian is required for all contestants under eighteen years of age. The signing of the agreement is a condition of participation in the Texas Salsa Championship. | |
